Goal: To provide support services for Aftercare
|

Objectives:
|
a) Learn how to be self-sufficient
b) Provide job readiness
c) Teachings of how to stabilize in the work force
d) Will be responsible for cleaning, doing laundry and cooking
e) Will register for IL classes through MCCS
f) Community mentors will be assigned
g) Will have a provider call list and will be trained to make all appointments
h) Will be specifically trained on time management
i) Will be responsible for setting up checking and savings accounts
j) Learn the necessary skills to transition into society
k) A performance algorithm will be put into place to determine whether the
resident is totally ready to be self supportive

a) Annual physical
b) Dental every 6 months
c) Drug and alcohol counseling
d) Sex offender counseling and therapy
e) Educational & vocational studies
f) Monthly clothing allowances
g) Personal maintenance services
h) Transportation to school, work, approved home and supervised visits
